Description: The day a patient leaves the hospital can be an exciting one—or a scary one—for the patient and his or her loved ones, given the potential for side effects, a slow recovery or even hospital readmission. This course covers the important role the case manager plays in discharge planning, by creating the best circumstances for recovery and support. The three course modules describe key elements of
discharge planning—including the philosophies and standards for such plans, the best practices for creating a patient-centered plan, and the external forces that affect even the best-laid plans.

Business goals: After completing this course, case managers can reasonably forecast:
» Maximizing the patent-centered approach to discharge planning by 20 percent
» Improving effectiveness of patient leveling by 10 percent
