Description: The purpose of Improving Outcomes in Palliative & EOL Care is to educate case managers so that they can offer clinical management based upon the latest evidence-based guidelines and standards of care for palliative and EOL care patients.
The course covers:
» Economic, regulatory and reimbursement impact of EOL care, as well as its ethical and legal principles, its care planning and symptom support and decision support and resource management

Business goals: When the CM team completes the PCEOL course, case managers can reasonably forecast:
» Increase in knowledge-on-topic for patients, families and the care team on the subject of palliative and EOL care by 40 percent
» Increase in completion of advance care planning by 40 percent
» Increase in patient admissions receiving a palliative care specialty consult to 10 percent
» Improvements in wound care claims by 10 percent
