Module B: Healthcare Analytics and Metrics for Success

CE Credits: 1.5

This module explores the various ways that patients’ needs, in terms of population health, can be boosted through the right use of technology. It begins by looking at the benefits of correct—and careful—use of electronic health records. It then explores how good software can help consolidate information (from claims to revenue and patient records) and offer data analysis about patient outcomes on a population level. Last, the module looks at the advances in health information technology (HIT)—from appointment-making technology to blood pressure cuffs with telemonitoring capabilities—that can help patients and healthcare providers stay connected while patients manage a chronic condition.

Learning objectives: After completing this module case managers will be able to:
» Describe melding information technology into population health management
» Discuss the benefits, challenges and threats associated with electronic health records (EHRs)
» Outline software applications to measure and document changes in population health management
» Review ways in which provider organizations can use population-level analyses for performance improvement
» Provide updates on patient technology that improves care

Advance your professional practice: As vital partners in a patient’s care, CMs need to understand health information technology (HIT) and the new tools and capabilities impacting care delivery, so that we can support patients in their ongoing care and help our healthcare organization streamline its systems.

Improving organizational performance: Advances in health information technology (HIT) can enable all members of the transdisciplinary team to be consistent and reinforce the care, educational points and progress with patients. When we can connect effectively with patients when they are away from a hospital or doctors’ offices, we can help them better manage their care and avoid unnecessary doctor visits or hospitalizations.
