Cardiovascular Risk: Coordination of Care
The Case Manager has an important task of coordinating ‘next step’ plans with patients who have had a myocardial infarction. Treatments, benefits, education and indentifying and clarifying the roles of individuals on the team are some these tasks. Specific educational needs for your patient as well as their family takes center stage to assure health and wellness is maintained.

Rheumatoid Arthritis: Adherence
Using correct and user-friendly terminology in the early stages of a partnership with your patients will help to promote open communications, adherence to the management plan and development of the self-management skills needed by your patient to understand and apply what is learned. Using open-ended questions to elicit that understanding and the concerns about rheumatoid arthritis and its treatment helps to educate your patient as well.

“Guidelines for Hand Hygiene”
Infection prevention begins with our hands. While we all know the proper and routine hand hygiene actions, do we really follow through each and every time we should be keeping our hands clean? Correctly using the alcohol-based hand rubs or washing with soap and water for at least 15 seconds are basics that everyone should know and do. The CDC has their National Handwashing guidelines available to reacquaint us all. An ounce of prevention is worth a pound of cure.
