There is precaution and an alert that patients who have either severe HF or have symptoms of HF (may not be severe) should not use this drug. The safety information also states there may be increased risk for patients on nitrates or insulin. Watch for Athena Forum’s new course on HF due online in September.

Obesity: Assessments with BMI The BMI is a tool used to assess overweight and obesity and monitor changes in body weight. BMI has its limitations because it does not measure body fat or muscle directly. A BMI calculator is available using both pounds and metrics through the National Heart Lung and Blood Institute (NHLBI). See Athena Forum’s appendix for BMI calculator. Two people can have the same BMI but different body fat percentages. A bodybuilder with a large muscle mass and low percentage of body fat may have the same BMI as a person who has more body fat.

HHS.gov: Electronic Medical Records

The HHS announced the final rules for making electronic medical records become more of a reality. “Under the Health Information Technology for Economic and Clinical Health Act (HITECH), federal incentive payments will be available to doctors and hospitals when they adopt EHRs and demonstrate use in ways that can improve quality, safety and effectiveness of care.” (CMS, July, 2010). This is exciting news and helps to move forward opportunities to both streamline information flow, and provide detailed patient information when it is needed to the healthcare team.

There will be a certification process along with standards to be met to achieve ‘eligible’ status. This will bring consistency & demonstrate “meaningful use” as defined by CMS.

The Affordable Care Act’s Impact on Chronic Care

With the signing of the Affordable Care Act into law, the U.S. healthcare system faces almost immediate – and substantial – transformation.

Among the numerous health policy implications, the law requires health plans to ensure quality of care and improve patient outcomes by means of quality reporting, effective case management, care coordination, chronic disease management, patient-centered education, medical error reduction, and implementation of wellness and health promotion activities.
The new law also raises a number of other policy issues for providers. In 2012, for instance, hospitals and physicians are invited to provide leadership in voluntary Accountable Care Organizations (ACOs) with the opportunity to share in cost savings from improved patient care management.

The Jefferson School of Population Health’s online Master of Science in Chronic Care Management (MS-CCM) program prepares leaders to critically analyze, design, implement and evaluate systems to improve chronic illness prevention and care management across various health care settings and populations.
