What’s the Plan – for Asthma Management? An Asthma Action Plan is one of the keys that patients and families need to best manage this chronic condition. As a Case Manager you need to ask patients when their Asthma Plan was established. When was the last time they reviewed the plan with their health care provider? Of course if they do not have a plan, the importance of an Action Plan and what should be included in the plan needs to occur. September celebrates ‘Recovery…’. Recovery month is a time for recognizing the efforts, energy and support that is involved in treatment of those dealing with substance abuse. Individuals find success through their own hard work, and the support of their families and professionals. It is not just a time of reflection, but a time to educate that public that ‘addiction is a treatable and recovery is possible.
