Discharge Planning — Reducing Readmissions That seems to be the mantra from our providers and payers – and many companies are poised to help in achieving that goal. As the Case Manager you are the one designated to coordinate the discharge that, in fact, provides what is necessary for patients to be successful at home and not return as ‘a re-admission’.

Women's Health & Osteoporosis This week on October 20, 2010 is World Osteoporosis Day which is supported by the International Osteoporosis Foundation. This is a great opportunity for all of us to take a minute to review the importance of early detection both for our patients, our family members and ourselves.

CDC.gov: Life expectancy better for Hispanic population.

Is it the close family ties of a culture? That is one of the comments from Dr. Elizabeth Arias of the Division of Vital Statistics. The CDC reported that Hispanics in the United States now have a longer life expectancy than African Americans and non-Hispanic whites. “The Hispanic population has a life expectancy advantage at birth of 2.5 years over the non-Hispanic white population and 7.7 years over the non-Hispanic black population. The reasons behind the lower mortality are not known.” (released October 13, 2010 www.cdc.gov)
