UM: Hospital Case Manager Deal with Quality Issues Every Day As hospitals and healthcare providers deal with quality improvement programs and how to measure quality, the Case Manager is in an ideal position to look at both quality and safety initiatives.
“The hospital case manager is continuously monitoring and evaluating the patient's care from several perspectives. Most important among monitoring activities related to quality of care are ensuring that:
• Interventions and procedures are relevant and correct for the patient's diagnosis and condition
• Treatments are effective and indicate desired and cost-effective outcomes “

COPD: Tools for Patient Management The American Lung Association offers tools for people with COPD in managing their condition. These tools are divided into three parts: Management; Action Plan; and Report Card. By providing people with a clear method to organize their treatment plan, understand symptoms and to track events can decrease some of the anxiety associated with COPD.

Details of items that can be included in a management and action plan are included in Athena Forum COPD course: “There are several preventive measures, overall health maintenance and safety measures that patients with COPD will need to understand in order to either prevent or minimize exacerbations of their illness.”

CDC.GOV: What about antibiotics? Patient education tools made available by CDC

As winter colds and sniffles begin to appear, the CDC has an informative campaign called: Get Smart About Antibiotics? The CDC is using this time as a reminder that misuse of antibiotics is a health safety issue and contributes to antibiotic resistance. To be smart about antibiotics everyone needs to remember 3 basic things:
• Don’t skip doses and take your medicine as prescribed. Antibiotics are most effective when taken as prescribed.
• Don’t save antibiotics. The drug is meant for a particular infection at that time. Don’t use leftover medicine. Taking the wrong drug can delay the appropriate treatment and your infection might get worse.
• Don’t take antibiotics prescribed for others. Only a health care professional can determine the right treatment for your infection. www.cdc.gov – Nov., 2010)
