Goal of UC Treatment, Remission, Mucosal Healing

Case managers recognize the primary goal of Ulcerative Colitis (UC) treatment is the achievement and maintenance of clinical and endoscopic remission. The gastrointestinal advisory panel of the FDA offers definitions for remission as an absence of inflammatory symptoms and evidence of mucosal healing. Recent evidence supports the suggestion that there is a link between increased inflammation and the risk of colorectal cancer. This underscores the importance of preventing relapses. Newly devised high-dose regimens are offering additional therapeutic options.

Listening is Key to MI Technique

Motivational Interviewing (MI) recognizes the value of patient responsibility and incorporates principles that support a patient's ability to change lifestyle.
MI is:

  • guiding more than directing

  • Dancing rather than wrestling

  • Listening at least as much as telling

The overall "spirit" of MI has been described as collaborative, evocative and honoring of patient autonomy. The techniques are designed to create an atmosphere in which the patient can decide what and when to change. Listening and soliciting the person's input are key aspects of MI.

CMS Incentives to Decrease HACs

Effective with discharges on or after October 1, 2008, CMS reduced annual inflation payments of 42 specific quality measures. The resulting payment reduction in 2010 was 2%. A reduction of 2% may sound small but it equals millions of dollars lost to the hospital. The quality measures are meant to give hospitals incentives to reduce serious adverse events during hospital stays that are preventable and to provide consumers with data to help them make informed decisions. Commercial payers have followed suit with pay for performance initiatives. The measures are available on the CMS website.

Partnership for Patients aims to save 60,000 lives

The HHS is allocating as much as $1 billion in federal funds to support its Partnership for Patients program, which aims to save 60,000 lives in three years by cutting rates of adverse events and medical errors. HHS Secretary Kathleen Sebelius said health IT will be key to meeting the goals of the program, which so far has received support from at least 500 health groups.
