Communication between the hospital case manager and either the ED physician, the physician on-call, or the patient's attending physician remains the best practice for ensuring that an initial assessment confirms the patient's need for acute care, treatment and service.

Not all patients require case management so hospitals and case managers must first establish who CM customers are.

The hospital's patient selection criteria for case management have to reliably identify admissions that will most benefit from CM services.

  Improving Patient Care...  

Standards of Care: Monitoring

Assessment doesn't end with admission. The case manager continuously monitors the patient to determine:

  • If the patient is receiving safe, quality, cost-effective care
  • If care plan goals are being achieved
  • Are the goals still appropriate and realistic
  • Do changes need to be made in the plan
  • Is the transitional plan still valid

Case managers should develop a routine for monitoring their patients' progress toward care plan goals.
