CV Risk: Circulatory Subsystems

The circulatory system includes four subsystems:

  • Arterial circulation - Arteries are the blood vessels that carry the oxygenated blood away from the heart.
  • Venous circulation - Veins are blood vessels that carry blood to the heart.
  • Capillary circulation is where oxygen, nutrients, and waste pass between the blood and the tissues of the body. Capillaries connect the arterial and venous circulatory subsystems.
  • Pulmonary circulation includes the movement of venous blood from the right side of the heart to the lungs and the arterial blood from the lungs back to the left side of the heart into the left atrium.

CVA Care Oversight

The National Stroke Association states that 25% of stroke survivors will have minor impairments, and up to 40% will have moderate to severe impairments necessitating treatment and rehabilitation services.

The need for Case Management to provide coordination of services, resource management and care oversight is obvious.

CV Risk: Reynolds Risk Score

By adding parental history of a heart attack before age 60 and C-reactive protein levels to the five traditional risk factors, age, hypertension, tobacco use, diabetes and hyperlipidemia, researchers developed an algorithm called the Reynolds Risk Score which led them to more accurately classify women previously considered at intermediate risk into higher- or lower-risk categories.

Better risk stratification has helped clinicians to prescribe preventive therapies to maximize treatment benefits and minimize costs.
