Exponential Growth in EOL Care

US hospice founded in the early 1970s has evolved from primarily volunteer run, community-governed endeavors to a multimillion dollar industry.

The exponential growth of palliative care in the US was fueled by the expansion of Medicare to include a hospice benefit and as a result of the recognition of the unique skills required to manage the needs of seriously ill patients.

Yet, overall hospice care remains underutilized and broader applications of palliative care in non-terminal patients are largely untapped.

Pivotal CM Role in EOL Care

"Case Managers are pivotal in facilitating smooth transitions in care from a curative to a palliative trajectory of care. They are essential members of the Palliative Care team.

"As patients and families often become overwhelmed in the palliative care environment, Case Managers provide essential support and guidance."

  Improving Patient Care...

EOL Teams Use Holistic Approach

Through the interdisciplinary team palliative care ensures that people are viewed in their entirety, not as collections of organs and medical problems.

This is in contrast to the approach frequently used in the delivery of care where a list of medical problems based on discrete organ systems and diseases is generated and used as a checklist to formulate treatment(s). This approach is ineffective when there are multiple diseases and serious conditions interacting and influencing one another.
