DP: Stress Related to
Leaving the Hospital

Patients often perceive leaving the hospital as the most stressful event of the hospital experience because they:

  • Feel unprepared to leave the hospital
  • Are stressed over insurance coverage
  • Don't know what is happening after they are discharged
  • Have been told that a home health nurse is to visit them but not when and why
  • Are being transferred to a sub-acute care facility, but have not been told why or where

  Excellence in Case Mgmt  

DP: ACMA Standards

Two of ACMA's standards, particularly applicable to patient-centered discharge planning are:

  • Communication
  • Patient advocacy

Together they provide an excellent check-list for discharge or transitional care planning and guide incorporation of the unique needs of each patient/family in the planning process.

A case manager works collaboratively with patients, families and a multidisciplinary team.

They are jointly accountable for measurable outcomes that are cost effective and reflect patient preferences and values.

Value-based purchasing (VBP) is all about rewarding providers for the value of their contributions to quality and efficient care that leads to better health outcomes: right care for the right patient at the right time. It shifts payment from volume to quality, efficient care. The goal is to transform Medicare from a passive payer of claims to an active purchaser of care.
