HF: Etiology

75% of HF cases are due to either ischemic heart disease or to hypertension. The balance of HF cases result from other causes.

Regardless of the etiology, odds of developing HF increase in individuals who have one of more of these conditions or risk factors.

Recognizing and treating the etiology can prevent or improve heart failure.

HF: Difficulties with Management

Self-care management is a decision-making process. Factors that interfere with self-care management include those that complicate the situation and interfere with the ability to make decisions. Virtually all individuals with HF have co-morbidities or other contributing factors which present both clinicians and patients with difficulties in managing HF.

The presence of co-morbidities contributes to difficulties in self-care in 4 major areas:

  • Medication adherence
  • Dietary adherence
  • Symptom monitoring
  • Decision-making about how to manage multiple conditions

HF: Preventing ADEs

HF patients often require complex medication regimens. Prescribed treatments are only effective it they are followed. Lack of patient adherence may adversely affect patient outcomes. Health care professionals should make every effort to obtain patient understanding and participation in prescribed treatments.

Many elderly patients are discharged with completely new sets of medications. Plus they have medications at home. Patients may assume that they are supposed to take both the old and new medications, resulting in adverse drug events (ADEs). Assure discharge instructions are clearly understood.
