Sepsis: Empowered CM Support the TD Team

RN and Social Worker case managers, both in the hospital and managed care settings interact with one another and with patients and their families concerning sepsis. Through these ongoing relationships, there is a great deal that CMs can offer in:

  • Identifying at risk individuals
  • Recognizing the existence of precipitating
  • Participating in transdisciplinary teams

By gaining further familiarity with the definition, natural history and pathophysiology of sepsis, CMs are empowered to be aware of early warning signs.

Sepsis: Just the Facts

Sepsis can be caused by an infection in virtually any part of the body.

Co-morbid conditions like cancer or diabetes can predispose patients to sepsis.

Surgical interventions are also a potentially precipitating event associated with sepsis.

In about 20% of sepsis cases the source of the sepsis can never be found.
