CV Risk: Embolisms

A blood clot that forms in a blood vessel or within the heart is called a thrombus. It can cause partial or complete interruption in blood flow.

A thrombus that travels from the blood vessel or heart to another location in the body is called an embolus, and the disorder, an embolism. An embolus that occurs in the lungs is called a pulmonary embolism.

Sometimes, a piece of atherosclerotic plaque, small pieces of tumor or fat, air, amniotic fluid, or other materials can behave like an embolus.

CV Risk: CM Coordinates "Next Steps"

Up to 40% of stroke survivors will have moderate to severe impairments necessitating treatment and rehabilitation. Clearly the CM will need to provide coordination of services, resource management and care oversight.

Coordination of care is also a CM function when working with patients who survive an MI. The CM has the task of coordinating 'next step' plans regarding treatment; obtaining benefit information; clarification of the individual roles of the team; and overseeing the educational needs for the patient and family.

UM Influences Quality

The HCM tracks the activities of the team and advocates for the patient in an effort to promote appropriate, efficient, quality, cost-effective care

At the same time the HCM monitors for over-utilization/under-utilization of resources (duplication of tests, timely results).

By being informed of patient progress toward goals the team members can make timely decisions pertaining to the appropriateness of care and change interventions and treatments to more effective ones based on the clinical status of the patient supported by objective data (results of tests and procedures).
