Principles of CM: Phases of HCM

Hospital case management (HCM) itself is typically an episodic process:

  1. Access-to-care, followed by
  2. Care coordination or throughput, and concluding with
  3. Patient discharge or transition to another level of care.

Access-to-care includes evaluating the patient for case management needs. Throughput involves the patient undergoing diagnostic tests or surgery. The core elements of care coordination should be viewed as interrelated activities in an integrated process. The final phase is discharge home or transition to a lower level of care.

Principles of CM: Keys to Success

Performance measurement: the ongoing monitoring and reporting of program accomplishments, particularly progress toward pre-established goals. It is typically conducted by program or agency management.

Program evaluations: individual systematic studies conducted periodically to assess how well a program is working. Evaluations can be conducted by experts external to the program and by program managers.

It is important to build in pre-determined program assessment points where everyone can step back to take a look at what the program has been accomplishing, what direction it has taken, and if it is working the way the planners intended it to work.

Principles of CM: Evaluating Care

Evaluation is the process of determining the patient's progress toward attainment of expected outcomes in the CM plan.

Evaluation questions the HCM might ask:

  • Is care progressing in line with the care plan?
  • Is care at the appropriate level of care?
  • Have any reimbursement issues or safety, legal, or ethical risks been encountered?
  • If the goals are not being met, what has caused the delay?
  • Does the care plan need modifications in order for the patient to make more timely progress toward goals?
