Asthma 2.0: Building a Partnership

Because effective asthma management requires active participation by the patient, aim to build a partnership with the patient from the time of the initial diagnosis.

It is especially important for the principal asthma provider to take the time to introduce key educational messages, emphasize the importance of self-management, and stress the importance of collaboration between the patient and the healthcare team.

The principal provider should also negotiate agreements with the patient; other members of the healthcare team can then reinforce and expand on those messages. Communication and coordination within the team are important.

Asthma 2.0: Determine Reversibility of Airflow Obstruction

In asthma, airflow obstruction is predominantly reversible, whereas in chronic obstructive pulmonary disease, it is generally irreversible (or only partially reversible). In asthma, the degree of reversibility correlates well with the level of underlying inflammation.

To determine reversibility, the patient undergoes spirometry testing before and after inhaling a short-acting bronchodilator medication, typically albuterol. Improved spirometry readings after taking the medication indicate the airflow obstruction is reversible.

According to American Thoracic Society/European Respiratory Society guidelines, an increase in FEV1 of more than 200 mL and that is at least 12% above the baseline (pre-bronchodilator) measure indicates significant reversibility.
