Pain Management: Use Multimodal Approach

Pharmacotherapy for pain includes a spectrum of choices including:

  • Non-opioids
  • Opioids
  • Adjuvant analgesic medications

Consider multimodal (the use of more than one medication or class of medications) to achieve maximal efficacy, while minimizing potential side-effects through careful selection.

When selecting pharmacotherapy it is important to keep in mind that some pain medications demonstrate greater efficacy for specific pain disorders.

Pain Management: Palliative Care
and Hospice

Advance care planning for people with cancer optimally begins soon after diagnosis. The discussions should include options regarding palliative care and hospice.

People with cancer make up approximately 41% of the hospice admissions. A disturbing trend is the median length of stay in hospice is just 20 days. This trend suggests that advance care planning:

  • Is not taking place early in the course of the disease
  • That the ability of health care providers to prognosticate is poor
  • That referrals are made too late
It may also reflect denial on the part of professionals, patients, or their families.

Pain Management: Acute Pain Categories

Three major categories, each with distinct characteristics, provide the etiology of acute pain. These are:

  • Neuropathic - Damage or destruction of a nerve, described as shooting pain
  • Somatic - Damage to skin and body wall, described as aching pain
  • Visceral - Damage to bodily organs, described as vague, diffuse pain
