  • The Framingham Heart Study suggests men have a 49% lifetime risk of coronary heart disease (CHD)
  • Over 70% of sudden cardiac events occur in men.
  • Major risk factors include high blood pressure, high cholesterol, tobacco use, diabetes, physical inactivity, and poor nutrition.
  • In a large study of BP treatment, an average reduction of 12 to 13 mm Hg in systolic blood pressure was associated with a 21% reduction in CHD, 37% reduction in stroke, and 13% reduction in all-cause mortality rates.
  • A 10% decrease in total cholesterol may reduce CHD by 30%

CV Risk: Can The Patient Adapt?

The Case Manager (CM) has the opportunity to both identify persons with cardio-vascular disease and those at risk for cardiac problems within their caseloads, regardless of the originating referral for Case Management services.

In the assessment process, the CM will learn those issues that may impact a person's ability to either adopt or adapt to a healthy lifestyle.
