EOL and Palliative Care:
What is Hope?

EOL and Palliative Care:
When Patient Needs Change

For the CM the goal of Palliative Care is genuine coordination of care across settings. This is ensured through regular and high quality communication among providers and with patients and families at times of:

  • Changing needs
  • Transitions of care
The provision of effective continuity of care utilizes all the techniques of case management.

EOL and Palliative Care: What It Is

Hospice care is always palliative, but not all palliative care is hospice care.

Palliative care is specialized medical care for people with serious illnesses. It is focused on providing relief from the symptoms, pain and stress of a serious illness.

The goal is to improve quality of life for the patient and the family. Palliative care is provided by a transdisciplinary team and other specialists who work together to provide an extra layer of support.

Palliative care is appropriate at any age and stage in a serious illness and can be provided together with curative treatment.
