Sepsis Care: Mysterious Pathogenesis

Sepsis arises unpredictably and can progress rapidly. Due to the diversity of microbial agents which cause sepsis, its pathogenesis as well as laboratory and clinical hallmarks are not uniformly established.

Sepsis begins when an infection such as pneumonia, bacterial meningitis or food poisoning enters the bloodstream. The body responds by calling in an overwhelming, system-wide counterattack. This immune response, which is sepsis, often proves as devastating as the infection itself.

Patient Transitions

For the CM the goal is genuine coordination of care across settings. This is ensured through regular and high quality communication among providers and with patients and families at times of:

  • Changing needs
  • Transitions of care

The provision of effective continuity of care utilizes the techniques of case management.

Inadequate planning for transitions of care results in increased costs and avoidable readmissions.

Sepsis Care: Inflammation Impairs Blood Flow

Immune chemicals released into the blood to combat the infection trigger widespread inflammation, which leads to:

  • Blood clots
  • Leaky vessels

This process results in impaired blood flow, which damages the body's organs by depriving them of nutrients and oxygen.

Due to the interruption of oxygen and nutrients to tissues, sepsis is a medical emergency on the same level as a myocardial infarction or a cerebrovascular accident.
