Abuse: Dimensions of DV
Cross-Sector Care

CMs guide patients through care processes, generally considered a continuous integration of dimensions both:

  • Medical
  • Social

Cross-sector care has distinct characteristics including that it:

  • Is commonly patient-centered and holistic
  • Takes patient empowerment into account
The focus of intervention is not limited to the hospital, often extending into the community. As population health and ACOs continue to become the care delivery mechanisms of choice, the focus of case management across levels of care will follow suit.

Abuse: Encouraging Candor and
Truth from Children

"It is critical for all those working with children to be alert to any child's comments or actions that may suggest that the child has experienced abuse or neglect.

"It is also important to keep any interviewing to a minimum, leaving this to people who are specially trained in the interviewing of neglected or abused children.

"Stay calm, do not show surprise regardless of what the child may say. Listen attentively and supportively."

Abuse: Identify EA Resources

As in other conditions when referring patients to services and specialists, providers screening for elder abuse are not expected to have all the answers. CMs should plan ahead of time to identify local resources and make contact with support services.

With these and other resources in place, CMs can more readily make a referral upon disclosure of abuse, including referrals in which providers give victims/survivors information so that individuals can make the best decision for themselves.
