AF News: ACO Beacon Health Scores High with AF

EMHS Beacon Health, a successful ACO eight-hospital system in Maine is an Athena Forum client. EMHS Beacon Health has 65 nurse care coordinators supporting 100,000 patients in a statewide network serving some of the most rural areas in the nation, says Tori Gaetani, RN, EMHS Beacon Health's director of care coordination.

Care coordinators meet with patients, call patients at home and work with physicians and other care providers. Last fall, CMS recognized EMHS Beacon Health for delivering the fifth highest quality scores of the initial 19 pioneer ACOs and getting high marks in patient satisfaction and engagement.

DM: The "Keep Healthy" Challenge

"ACOs like Beacon Health are charged with keeping patients healthy rather than treating them when sick. As soon as a health system becomes an ACO, they accept responsibility to care for patients regardless of location, meaning they need to care for patients who are not even in the hospital in many cases."

Case management of patients with DM has been shown to significantly improve health and psychosocial outcomes in both primary care and community settings. By improving diabetes management in their patient population, CMs can help reduce the tremendous burden of this common disease on individuals and families.


DM: Engaging the Patient for Success

Caring for patients outside the hospital, particularly in remote areas, poses challenges. To help engage patients, CMs at Beacon Health are offered training on Athena Forum in motivational interviewing. It's critical that care coordinators have a thorough understanding about how to engage patients in their care.

Instead of "pushing" the patient to change, which can result in resistance, the CM elicits from the person their own motivations for making changes in the interest of their health. The tone of MI is nonjudgmental and encouraging. The patient decides what and when to change.
