Improving HF Management

This fall, the federal Centers for Medicare & Medicaid Services will penalize 2,592 hospitals a combined total of $420 million for having too many readmissions for heart failure and four other conditions within 30 days of discharge. Those hospitals will receive lower payments for every Medicare patient who stays in the hospital - readmitted or not - starting in October, Kaiser Health News reported. For those hospitals seeking to avoid such penalties next year, they might consider enrolling their case managers and social workers in Athena Forum's new 2.0 version of "Improving Outcomes in Heart Failure Management" (publishing soon).

Heart Failure: Telling vs. Engaging

It is vital to keep in mind that each person has an internal set of pros and cons in the choices they make. It is important to determine what those pros and cons are in order to support patients in accomplishing their health related goals.

Simply telling people about what they should do is not particularly effective. Generally, patients need assistance in taking responsibility for their outcomes and in self-managing their conditions.
