Tips from our featured course

HF: CM Can Reduce the Impact of Heart Failure

As care needs become more complex, the number of transdisciplinary participants tends to increase.

"At the end of the day all individuals can only be accountable for their own actions. This includes the way in which case managers engage, assess, intervene, advocate and plan for the patients and families who are impacted daily by HF.

There is no doubt of case managers' tremendous opportunity to greatly influence the transdisciplinary care coordination for this rapidly growing segment of the population."

Heart Failure: Value-based Health Management

As healthcare shifts from episodic care to value-based health management, hospitals must take a more proactive approach to engaging patients, especially individuals with chronic disease(s) who represent a higher risk for organizations pursuing a risk-sharing approach.

Organizations have in effect standards, guidelines and indicators for HF care and have ramped up efforts to promote:

  • Better outcomes
  • Patient safety
  • Cost-effective care

Hospitals are a logical focus for efforts to reduce costs, particularly those associated with potentially preventable admissions.
