Discharge Planning: Critical for Safe,
Cost-Effective Care Transitions
"As healthcare costs continue to increase and the United States healthcare quality decreases compared nationally, we find ourselves needing to create cost savings from within healthcare to prevent bankrupting the future of our children with the mounting costs of Medicare. One of the ways to ensure that patients stay healthy and independent is to provide the safest and most comprehensive discharge planning possible."
