Community CM Programs Reduce Acute
Encounters for Uninsured
HCMs must develop creative discharge plans and transitions of care to prevent readmissions for the uninsured population.
This is accomplished through many avenues, including:
- Researching community resources
- Working with colleagues to identify funding opportunities
- Providing medication education sessions
- Assisting patients to apply for pharmacy assistance programs
- Connecting the patient with available community-based case management programs
Uninsured patients' participation in community case management programs has been associated with statistically significant reductions in both acute outpatient encounters and inpatient admissions along with a concomitant increase in primary care clinic visits.
